The following form was developed by non-for-profit organizations in the city of Chicago for two main purposes:

1. The following questions will help you have a record and organize your information and immigration journey.
2. Legal aid staff from partner organizations will review your responses, advice you of your rights and legal immigration processes, and review forms of immigration relief (if any are available to you).

You have registered for an appointment at the Centralized Immigration Legal Services Center. You must complete this form to the best of your ability at least 24 hours prior to attending your appointment.

By completing this form, you understand that the information you provide and any information you discuss with an attorney or legal representative during a screening session will be used to determine eligibility for immigration relief. You agree to not withheld any information during the screening session and that the information discussed is true and accurate to the best of your knowledge.
